JOB TITLE:                                  Public Health Nurse II Communicable Disease Coordinator

ESSENTIAL JOB DUTIES:  Coordinator of Communicable Disease program: manage the communicable disease program to prevent further spread of diseases; assess, evaluate, plan and coordinate clinical and community services in the areas of communicable disease, establishing program goals, policies, procedures and guidelines in collaboration with Nursing Supervisor, Nursing Director and Medical Director; primary concentration of investigation, follow up and reporting of communicable diseases for Alamance County; serve in lead capacity for referrals/reports and investigation of communicable diseases; coordinate services in the clinic and community setting in the case of outbreak situations; consult and collaborate with the EPI team and the NC Epidemiology Section; complete reports as required by the NC Communicable Disease Law; serve as lead for Communicable Disease Reporting and Investigation (NCEDSS system); conduct monthly communicable disease program meetings; complete program contract addendums; make home visits as needed; work in communicable disease clinic and/or community settings to provide testing, vaccines, education and counseling; Serve as liaison between health department communicable disease program and community physicians, medical facilities, community agencies, etc. to enhance professional relationships and to achieve coordinated and cooperative efforts toward the control of communicable diseases; coordinate with the agency OSHA Coordinator to implement, update and evaluate the Infection Control Program for the agency;  serve as back up to the agency OSHA/Infection Control Nurse; Serve as a resource person for communicable diseases, OSHA bloodborne pathogens for health department staff, the public, and other county agencies; Consult with program state consultants as needed to review program operation and obtain guidance; staff emergency shelters as mandated by the State Public Health Office; Must be available by cell phone for 24-hour coverage for communicable disease reporting; Serve as the Adult Day Health Specialist for quarterly monitoring of area Adult Day Health Centers; and perform additional duties as required by the Nursing Supervisor, Nursing Director and Health Director. This position also works a minimum of 12 hours/week providing direct patient care to patients seeking clinical services.


ESSENTIAL KNOWLEDGE, SKILLS, AND ABILITIESConsiderable knowledge of and skill in the application of nursing theory, practices, principles, and techniques employed in the field of public health and related programs; considerable knowledge of and ability to apply the principles and practices of public health; considerable knowledge of available resources and organizations and the ability to coordinate these as needed; general knowledge of current social and economic problems relating to public health.  Ability to plan, coordinate, and supervise the work of others; ability to deal tactfully with others and to exercise good judgment in appraising situations and making decisions; ability to secure the cooperation of clients, to elicit needed information, and to maintain effective working relationships; ability to record accurately services rendered and to interpret and explain records, reports, and medical instructions.


ESSENTIAL PHYSICAL ABILITIES: Medium type work (exerting up to 50 lbs. of force) with physical abilities that include: climbing, kneeling, crouching, reaching, standing (for sustained periods of time), walking, pushing, pulling, lifting, fingering, grasping, feeling, talking and hearing.  Subject to repetitive motion of the wrist, hands, or fingers.  Require close visual acuity for job duties such as transcription, extensive reading, operating computer terminal and measurement devices; and ability to work outside and inside extreme environmental conditions.  May be exposed to blood or body fluids in performance of job duties.


MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:  Graduation from a four-year college or university with a B. S. Degree in Nursing which includes in a Public Health Nursing rotation and one year of Public Health Nursing experience; or graduation from an accredited school of professional nursing and two years of professional nursing experience including one year in public health; or an equivalent combination of training and experience.


NECESSARY SPECIAL QUALIFICATIONS:  A current license to practice as a Registered Nurse in North Carolina by the N. C. Board of Nursing.  Valid N.C. driver's license and have access to a personal vehicle.


PREFERRED QUALIFICATIONS:  Knowledge and experience with Communicable Disease preferred. Fluent in both Spanish and English both oral and written.
